Covered Benefits

With OneCare (HMO SNP), you get total care. All your benefits are in one plan.

Our generous health care coverage includes:

  • $0 co-payment on brand-name drugs until total yearly drug costs reach $2,000

  • $0 co-payment on generic drugs until total yearly drug costs reach $2,000

  • Doctor or specialist visits - always at $0 cost

  • $0 cost medical transportation - 60 one-way taxi rides per year

  • Worldwide emergency health care coverage up
    to $5,000

Dental benefits include:

  • $0 co-payment on teeth cleaning twice a year

  • $0 co-payment on crowns, dentures, fillings,
    x-rays, root canals and more

Vision benefits include:

  • Vision exam every year - $0 cost

  • Every two years, up to $200 for eyeglasses (frames and lenses) or up to $200 for
    contact lenses
