COVID-19 Vaccine Member Health Rewards End December 31, 2023!

If you received your COVID-19 vaccine, please be patient. There may be delays in confirming your vaccination and mailing your COVID-19 Vaccine Member Health Rewards gift card(s).

1. Am I eligible for COVID-19 Vaccine Member Health Rewards?

The COVID-19 Vaccine Member Health Rewards program for CalOptima Health members ends December 31, 2023. CalOptima Health is following the latest Centers for Disease Control and Prevention guidance updated on April 19, 2023. To qualify, members must:

  • Be 6 months of age or older
  • Complete the first COVID-19 dose no later than June 30, 2023
  • Complete any additional recommended doses by December 31, 2023

Gift card guidelines:

  1. Members who began their first COVID-19 vaccine before April 19, 2023, are eligible to get 1 gift card per recommended dose, with a limit of up to 4 gift cards per person.
  2. If members begin their first vaccine dose after April 19, 2023, they may be eligible for 1 or more gift cards depending on the recommended number of doses based on their age.
  3. Members must be eligible with CalOptima Health on their vaccination date(s) to receive the gift card(s).
  4. Members in long-term care and participants in the CalOptima Health PACE are not eligible for the COVID-19 Vaccine Member Health Rewards program.

2. How can I get my health rewards?

Members do not need to submit anything to CalOptima Health. Gift cards will be sent after CalOptima Health confirms the status of your vaccinations through the California Immunization Registry. Your gift card will be sent to the mailing address CalOptima Health has on file for you. To make sure the correct address is on file, please log in to the CalOptima Health Member Portal or create an account. You can also call Customer Service to update your mailing address at 1-714-246-8500 or toll-free at 1-888-587-8088 (TTY 711), Monday through Friday, from 8 a.m. to 5:30 p.m.

3. When will I receive my health rewards gift card?

Due to many members participating in this program, the delivery of your gift card(s) may be delayed. You may get your gift cards for different doses at different times. Thank you for your patience.

4. What if my health rewards gift card is lost or stolen?

CalOptima Health is not responsible for lost or stolen gift cards. Gift cards will not be reissued. Please make sure CalOptima Health has your correct mailing address.

5. Is there anything else I should know about the COVID-19 Vaccine Member Health Rewards Program?

Gift cards cannot be used to purchase alcohol, tobacco or firearms. The gift cards have no cash value. Gift cards are available while supplies last. This health reward may end at any time without notice. CalOptima Health complies with applicable State and Federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability or sex.
