What is OneCare (HMO SNP)?

OneCare (HMO SNP) means total care. All your Medicare and Medi-Cal benefits are covered in one single plan making it easier to get the health care you need. With OneCare from CalOptima, you can choose from more than 2,800 physicians in Orange County who are ready to serve you.

We provide prescriptions, dental and vision coverage, and many other benefits, such as transportation to and from your medical appointments.

To be a member of CalOptima OneCare,
you must:

  • Live in Orange County

  • Be enrolled in Medi-Cal and Medicare
    Parts A and B

OneCare (HMO SNP) is a Coordinated Care plan with a Medicare Advantage contract and a contract with the California Medicaid program. Enrollment in OneCare (HMO SNP) depends on contract renewal. Eligible beneficiaries can enroll at any time. Eligible beneficiaries must have Part A and Part B to enroll in the plan. The benefit information herein is a brief summary, but not a comprehensive description of available benefits. Additional information about benefits is available to assist you in making a decision about your coverage. Benefits may change January 1, 2015. This is an advertisement; for more information contact the plan. The State of California pays the Medicare Part B premiums for full dual members of OneCare (HMO SNP). You must receive all routine care from plan providers except in emergent or urgent care situations or for out-of-area renal dialysis. If you obtain routine care from out-of-network providers neither Medicare nor OneCare will be responsible for the costs. Beneficiaries must use network pharmacies to access their prescription drug benefit except in non-routine circumstances and quantity limits and restrictions might apply. Availability of coverage beyond the end of the current contract year is not guaranteed.
