CalOptima Health PACE supports seniors age 55 and older who qualify for nursing home care but want to stay in their own homes.

Keeping You Active and Healthy

CalOptima Health PACE helps you live a more full and independent life while staying at home. Serving Medi-Cal and Medicare seniors for more than 10 years, PACE has review scores greater than 95%, according to CalOptima Health PACE’s I-SAT Participant Satisfaction Report (January 2024, pg. ii; conducted by Vital Research, an independent research firm). Eligible seniors from all Orange County ZIP codes are welcome to join.

Call Us

Call 1-844-999-PACE (7223) (TTY 1-714-468-1063) to tell us you’re interested. We’ll ask about your age, home, health and more to see if you qualify.

Meet With Our Team

Our health care team will visit you at home to learn more about your needs and check if you meet the state’s requirements.

Attend Your Enrollment Meeting

You’ll be invited to a meeting to learn about our PACE program benefits and fill out the forms, such as the enrollment agreement.

Get PACE Services

Congratulations — you’re enrolled! Our team will work with you to craft your care plan.

  • What will I do at the PACE Center?

    Depending on your needs, you could visit our PACE Center one to five times a week to:

    • Get comprehensive health care services
    • Enjoy healthy meals 
    • Join fun activities with other participants
  • How do I get to and from the PACE Center?

    Our friendly, reliable van drivers will bring you to and from our PACE Center and your home.
  • What is an interdisciplinary team (IDT)?

    Your IDT works together to give you comprehensive and coordinated care. It is made up of health care professionals, such as: 

    • Primary care providers
    • Specialists
    • Nurses
    • Pharmacists
    • Social workers
    • Dietitians
    • Physical therapists
    • Occupational therapists
    • Home care staff
    • Activity recreational staff
    • Transportation staff

    Our IDT works with you and your family to create an individual care plan (ICP), that is managed on a daily basis. By seeing you often at PACE, our IDT can see if your health changes and modify your care plan.

  • How do I pay for your PACE program?

    Our PACE program is funded by Medicare, Medi-Cal or both. Our team can help you understand your coverage and any out-of-pocket costs.
  • What if I am not satisfied with PACE?

    Our team strives to respond to your concerns and improve your experience with our PACE program. You can disenroll at any time, and we will help you move to new health care services.
