Take an active role in your health! CalOptima Health offers rewards to eligible members for completing important preventive screenings, tests or exams.

New for 2026: Online Health Reward Form

Medi-Cal and OneCare Complete (HMO D-SNP) members can now submit one simple online form to claim their health reward. (Paper forms are no longer required.) Fill out the online form at the link below after you complete your preventive screening, test or exam. Please note:

You must meet all health reward eligibility requirements to qualify for your reward
Each health reward may be earned once per calendar year, while funds last
Rewards cannot be used to buy alcohol, tobacco, firearms, gift cards or lottery tickets
The program may change or end without notice
Starting January 1, 2026, Medi-Cal members with other primary insurance are not eligible for member health rewards

When Will I Get My Reward?

It may take up to 90 days to process your reward after we confirm your visit has been completed
There are age limits unless your provider recommends a screening that’s not in the listed age ranges

Funds will be added to your OneCare flex card
