General requirements to be a CalOptima Health Provider
- Medi-Cal and Medicare enrolled
 - Board Certified
 - Adhere to CalOptima Health Policy and Procedures
 - Good standing with Regulatory Agency Requirements including but not limited to DHCS, CMS and DMHC
 - Provide services in Orange County and bordering cities
 - Meet network adequacy and access standards
 - CAQH enrolled/Credentialing review (as applicable)
- Credentialing must be renewed every three years, and the process to renew starts six months before your due date. Between credentialing cycles, we require up-to-date documents when renewing and notice of any change of status, address and/or carrier:
- State license
 - DEA
 - Malpractice insurance
 - Board certifications

 - Updating your CAQH information/submitting your credentialing application and requested support documents quickly is key to timely credentialing.
