How to Contract With CalOptima Health

Interested in becoming a contracted provider partner with CalOptima Health?

If you would like to play a role in strengthening Orange County’s health care delivery system, then becoming a CalOptima Health provider partner is for you.

Who do we serve?

CalOptima Health offers health insurance plans for low-income children, adults, seniors and people with disabilities in Orange County.

General requirements to be a CalOptima Health Provider

  • Medi-Cal and Medicare enrolled
  • Board Certified
  • Professional Liability of 1M per incident, 3M in aggregate
  • Commercial General Liability (CGL)/Automobile Liability of 1M per incident, 3M in aggregate
  • Cyber Security insurance of 1M per incident, 1M in aggregate
  • Be in good standing with regulatory agency requirements including but not limited to DHCS, CMS and DMHC
  • Provide services in Orange County or bordering cities (as may be needed)
  • Complete eCredentialing application and required forms
  • Complete: Offshore Attestation and Ownership & Disclosure

How to Become a Contracted Provider

Start by submitting your Letter of Interest to ProviderOnboarding@caloptima.org.   
Include the following:

  1. Legal Name and DBA if applicable
  2. National Provider Identifier (NPI)  
  3. Types of service(s) provided
  4. Practitioner's hospital/facility (ASC) privilege(s)
  5. Service Address(es)
  6. Proof of Medi-Cal/Medicare enrollment
  7. W-9 Form
  8. Administrator's contact information
