You’ll get PACE services at your home or at our PACE Center. Follow these steps to enroll:
Contact us to see if PACE is the right fit for you. We’ll learn about your health care needs and explore how our PACE program can help you, including Alternative Care Settings (ACS) and community-based doctors.
We’ll submit your application to the California Department of Health Care Services (DHCS) for review and approval.
Once approved by DHCS, we’ll help you finish the enrollment application.
Congratulations! You’re now a PACE participant and will start getting PACE services on the first of the following month.

Visit our PACE Center in Garden Grove — the first of its kind in Orange County. It offers our PACE participants medical care, home care, rehabilitation, activities and transportation.
