General requirements to be a CalOptima Health Provider
- Medi-Cal and Medicare enrolled
- Board Certified
- Maintain Professional Liability of 1M per incident, 3M in aggregate
- Maintain Commercial General Liability (CGL)/Automobile Liability of 1M per incident, 3M in aggregate
- Maintain Cyber Security insurance of 1M per incident, 1M in aggregate
- Be in good standing with regulatory agency requirements including but not limited to DHCS, CMS and DMHC
- Provide services in Orange County or bordering cities (as may be needed)
- Complete credentialing application and include required supplemental forms
- Complete Forms: Offshore Attestation and Ownership & Disclosure
